Honeywell HomMed Enhances LifeStream Management Suite withPersonal Emergency Response System (PERS) for Seamless Patient Care Transitions

SAN JOSE, Calif. — April 29, 2012 — Honeywell (NYSE: HON) -- HomMed, a global leader in telehealthand remote patient care, announced today the addition of a mobile telecare service to its product offering – a PersonalEmergency Response System (PERS) from MobileHelp – that allows users to summon emergency help anytime,anywhere, with the touch of a button. The PERS integrates with Honeywell's LifeStream Management Suite and improves the safety and security of patient care transitions.

 

A first for the telehealth industry, Honeywell’s LifeStream Management Suite now integrates telehealth and telecarepatient data on one dashboard. Designed to provide a broader picture of patients’ health as they move through care transitions, healthcare providers will have access to both remote monitoring data as well as emergency PERS alerts inone patient record.

The MobileHelp PERS offers enhanced user flexibility by providing the choice of a mobile unit, a base station (landline)unit, or both, as well as a pendant or wrist button. When the patient is at home, it works like a traditional medical alertsystem. Away from home, the small, hand-held unit can be carried or clipped to a belt, allowing patients the freedom tobe completely mobile. A push of the button automatically connects the user to an emergency operator, who can send help to the user’s exact location. Data from the PERS unit is then integrated back into the LifeStream Management Suiteas an alert in the patient record, providing an additional data point for care providers who are monitoring a patient’s health.

According to the Center for Medicare and Medicaid Services (CMS), nearly one in five patients discharged from thehospital is readmitted within 30 days, at a cost of over $26 billion per year. And according to the independent, not-for-profit organization, the Joint Commission, hospital readmissions are mainly a result of gaps in follow-up care.

 

To that end, Honeywell’s LifeStream Management Suite - the software platform providing the behind-the-scenes technology support for Honeywell’s devices – creates a technology- based “constant care” for patients as they move between care environments.

LifeStream Management Suite serves as the critical connection point between a patient using a remote monitoringdevice and their healthcare provider, while also streamlining and organizing patient data as it flows into the healthcare provider’s system, improving workflow efficiencies among healthcare providers.

 

Honeywell’s LifeStream Management Suite includes five software applications:

 

LifeStream™ Manager: As Honeywell’s leading healthcare-provider facing application, LifeStream Managerinteroperably manages all in-home patient monitors from a single dashboard, allowing one healthcare providerto effectively monitor up to 200 patients’ data at any one time. LifeStream Manager also allows healthcareproviders to customize which biometrics are collected from patients and when, increasing overall patientcompliance to levels as high as 90 percent. Other features include the ability to view indicator flags when real-time patient data falls out of acceptable ranges, immediately alerting healthcare providers to patients whosedata points indicate triage intervention is necessary.

 

LifeStream™ View: Providing secure access to patient information via the Internet, LifeStream View ensures patients, families and healthcare providers have the ability to access a current picture of the patient’s health status at any time – anywhere they have an internet connection. It also allows patient data points to beorganized into tabs and graphs that can be used to help identify trends in the patient’s overall health.
